Chronic Myeloid Leukemia (CML) is a slow growing leukemia or blood cancer that starts in the bone marrow and is characterized by the over production of abnormal or mutated white blood cells, specifically myeloid cells. These abnormal cells build up in the bone marrow, crowd out healthy blood cells, and can eventually enter the bloodstream and spread to other parts of the body, such as the spleen.
Causes of Chronic Myeloid Leukemia (CML)
The main cause of CML is mostly through genetic abnormality that a person has acquired improper diet and lifestyle during his lifetime (it is generally not inherited).
1) The Philadelphia Chromosome: The vast majority of people (around 90%) with CML have an abnormal chromosome called the Philadelphia chromosome (Ph chromosome). This is a result of a translocation (a swap of genetic material) between chromosome 9 and chromosome 22.
2) The BCR::ABL1 Gene: This translocation creates a new gene called BCR::ABL1, which produces an abnormal protein called tyrosine kinase. This protein signals the blood cells to proliferate and divide out of control.
Risk Factors of Chronic Myeloid Leukemia (CML)
1) Age: The risk increases with age, most commonly occurring in older adults.
2) Sex: It is slightly more common in males.
3) Radiation Exposure: Exposure to very high doses of radiation (like that from a nuclear accident or high-dose radiation therapy for other cancers) is a known but rare risk factor.
Phases of Chronic Myeloid Leukemia
It progresses through three phases and it affect both the symptoms and the treatment approach:
1) Chronic Phase: Generally, most patient are diagnosed in this phase. In this phase the leukemia is slow growing and there are typically less than 10% blast cells or immature leukemia cells in the blood or the bone marrow. Patient generally respond well to the treatment in this phase.
2) Accelerated Phase: This is more aggressive and has a higher percentage of blast cells (10 – 19%). Symptoms become more noticeable in this phase and the leukemia may not respond to the treatment.
3) Blast Phase (or Blast Phase): This is more aggressive and dangerous phase of Chronic Myeloid Leukemia (CML). It behaves like acute leukemia and is characterized by presence of more than 20% or more blast cells in the blood or the bone marrow.
Symptoms of Chronic Myeloid Leukemia
Many people with CML, particularly in the early stages, have no symptoms and the disease is often discovered during a routine blood test.
When symptoms do occur, they are often vague and can include:
1) Fatigue and Weakness (due to anemia, a shortage of red blood cells).
2) Weight loss and loss of appetite.
3) Fever and drenching night sweats.
4) Pain or a feeling of fullness below the ribs on the left side, caused by an enlarged spleen (splenomegaly).
5) Easy bruising or bleeding (due to a shortage of platelets).
6) Bone pain.
