What is Diabetes Mellitus?

Generally known as diabetes, diabetes mellitus is the condition in which your body fails to maintain your blood sugar levels properly. Normally, when you eat food, especially carb, your body breaks it down into sugar glucose, a form of sugar, to provide energy for your cells. In this respect, though, a hormone named insulin, which is produced in the pancreas, helps glucose penetrate into your cells.

Excess sugars in your blood are likely to cause health complications if not well managed over time, including harming your heart, kidneys, eyes, and nerves. Management of diabetes often includes a regulation of the blood sugar levels with diet, exercise, and medication.

What are types of Diabetes Mellitus?

There are three major kinds of Diabetes Mellitus, which might induce the other two, but they all have something alike; namely, high blood sugar.

  1. Type 1 Diabetes: At this point, your body immune system starts attacking some of the cells that are making insulin in the pancreas. Consequently, glucose cannot be introduced into cells without insulin; thus, it accumulates in the blood. Those with Type 1 will be required to be on insulin injections every day as their body stops its manufacture. It typically occurs at onset of the disease in children or a young adult but can start at any age.
  2. Type 2 Diabetes: It is the most common. It develops gradually. With type 2 diabetes, the body either doesn’t make enough insulin or the cells are insensitive to it. Glucose cannot enter the cells and glucose levels become high. Type 2 often comes from lifestyle habits that contribute to being overweight, lack of physical activity, or other things. It’s often treated with lifestyle changes, medication, and sometimes insulin.
  3. Gestational Diabetes: This type develops during pregnancy. Hormones produced by the placenta can cause the mother’s body to become less responsive to insulin, increasing blood sugar. While it tends to resolve after delivery, gestational diabetes can put the risk for Type 2 diabetes in both the mother and the child.

Colon, which is also popularly known as large intestine, is a part of digestive system. Large intestine or colon plays an important role in processing the digested food and absorbing water from the digested food material. The colon is almost five to six feet long and is further subdivided into multiple sections: 1) Ascending Colon. 2) Transverse Colon. 3) Descending Colon. 4) Sigmoid Colon.

Functions of Colon:

1) Water and electrolyte absorption: The colon absorbs water and salts from the digested food material and turns it into solid waste called stools.

2) Bacterial fermentation: The colon mucosal lining consist of beneficial bacteria that helps to break down certain undigested food material and further improves gut health.

What is Colon Cancer?

Colon cancer, which is also called as colorectal cancer or bowel cancer, is a type of cancer that originate or begins in the colon or the rectum. Colorectal cancer typically begins as small and benign (non-cancerous) growth called as polyp. This polyp typically over years accumulates additional mutations and turn into cancer if not removed.

Symptoms of Colon Cancer:

1) Changes in bowel habits, such as unexplained diarrhea or constipation.

2) Blood in the stool or rectal bleeding.

3) Unexplained Weight Loss.

4) Feeling of incomplete emptying of the bowel.

5) Fatigue or weakness.

What is Ayurveda?

Ayurveda is an ancient system of Indian Medicine and has originated in India over 5,000 years ago, Ayurveda emphasizes on holistic approach to health and strongly preaches that health of an individual relies over balance between mind, body, and spirit. The word “Ayurveda” comes from the Sanskrit word, “Ayur” (life) and “Veda” (knowledge or science), so Ayurveda means “the science of life”.

Key concepts of Ayurveda are:

a) Panchamahabhuta Siddhanta (Five Elements): Ayurveda strongly believes and preaches that everything in the universe is composed five basic elements. This five basic elements are also constituent building block of the human body. This five basic elements are:

1) Akasha (Space). 2) Vayu (Air). 3) Agni (Fire). 4) Jala (Water). 5) Prithvi (Earth).

b) Dosha Siddhanta (Constitutional Types): Ayurveda strongly preaches that the body consists of three doshas, these three doshas determines each individual physical and mental trait of an individual.

1) Vata (Vayu and Akasha): Vata controls bodily movement, breathing, and communication of the body.

2) Pitta (Agni and Jala): Pitta controls metabolism, digestion, and temperature of the body.

3) Kapha (Prithvi and Jala): Kapha is responsible for structure, immunity, and stability of the individual.

١)  ما هو القولون

يعتبر القولون جزءًا مهمًا من الجهاز الهضمي. يساعد القولون بشكل أساسي في معالجة الفضلات الناتجة عن المواد المهضومة. كما يساعد على امتصاص الماء والشوارد من المواد المهضومة. كما يعمل القولون على تحضير المواد المهضومة للتخلص منها. ويتكون القولون من عدة أجزاء هي كما يلي:

ا) الأعور: الأعور هو جزء من الأمعاء الغليظة حيث يتصل بالأمعاء الدقيقة. الزائدة الدودية متصلة بالأعور.

ب) القولون الصاعد: يقع على الجانب الأيمن من البطن ويمتد إلى الأعلى.

ج) القولون المستعرض: يقع القولون المستعرض في الجزء العلوي من القولون ويمتد أفقيًا.

د ) القولون الهابط: يقع على الجانب الأيسر من البطن ويمتد إلى الأسفل.

ر) القولون السيني: هو بنية على شكل حرف S ويؤدي إلى المستقيم.

يحتوي القولون على عدد كبير من البكتيريا المفيدة التي تساعد في تكسير الطعام غير المهضوم وإنتاج عناصر مهمة مثل فيتامين K يساعد القولون على تجميد الفضلات ويساعد في تكوين البراز.

۲) ما هو سرطان القولون

يشمل سرطان القولون، والذي يمكن أن يُطلق عليه أيضًا سرطان القولون والمستقيم، السرطان الذي ينشأ في القولون والمستقيم. سرطان القولون والمستقيم هو نوع من السرطان ينشأ أو يبدأ في الأمعاء الغليظة (القولون). يبدأ سرطان القولون والمستقيم عادةً على شكل كتل غير سرطانية وغير منسجمة من الخلايا تسمى السليلة. تكون هذه السليلة غير متحركة وتتكون في البطانة الداخلية للقولون أو المستقيم. مع مرور الوقت، قد تكتسب بعض السليلة طفرات إضافية ويمكن أن تؤدي إلى تحول سرطاني.

) عوامل الخطر لسرطان القولون والمستقيم ۳

لا يزال السبب الدقيق لسرطان القولون والمستقيم غير واضح ولكن هناك عوامل معينة تزيد من عوامل الخطر وهي:

ا) العمر: يتم تشخيص معظم المصابين بسرطان القولون والمستقيم فوق سن الخمسين عامًا ولكن من المعروف أن هذا المرض يمكن أن يحدث في أي عمر.

ب) العوامل الوراثية: يمكن أن تزيد الطفرات الجينية الموروثة مثل متلازمة لينش ومتلازمة السلائل الغدية العائلية (FAP) من خطر الإصابة بسرطان القولون والمستقيم.

ج) النظام الغذائي: يمكن أن يؤدي اتباع نظام غذائي غني باللحوم الحمراء واللحوم المصنعة ومنخفض الألياف إلى زيادة خطر الإصابة بسرطان القولون والمستقيم.

د) مرض التهاب الأمعاء: يمكن أن يزيد مرض التهاب الأمعاء مثل مرض كرون أو التهاب القولون التقرحي من احتمالية الإصابة بسرطان القولون والمستقيم.

۴) أعراض سرطان القولون

قد لا يسبب سرطان القولون في مراحله المبكرة أي أعراض، ولكن مع تقدم المرض قد تتضمن بعض الأعراض الشائعة ما يلي:

ا) تغيرات في عادات الأمعاء: قد تتجلى تغيرات عادات الأمعاء في الإسهال والإمساك الذي يستمر لأكثر من بضعة أيام. ومع تقدم المرض، قد تتفاقم هذه الأعراض تدريجيًا.

ب) النزيف عبر المستقيم: يمكن أن يتجلى سرطان القولون أيضًا في النزيف عبر المستقيم أو الدم في البراز. عادة ما يتم إجراء اختبار الدم الخفي في البراز (FBT) لتشخيص سرطان القولون.

ج) فقدان الوزن غير المبرر: فقدان أكثر من ١٠٪ من الوزن دون محاولة القيام بذلك يستحق المزيد من التحقيقات. يمكن أن يتجلى سرطان القولون في مرحلة متقدمة أيضًا في فقدان الوزن.

د) التعب أو الضعف: يمكن أن يكون الشعور المستمر بالضعف أو التعب علامة على سرطان القولون. لا يختفي هذا الشعور بالتعب أو الضعف حتى مع الراحة الكافية.

ر) ثقل في الأمعاء: قد يكون الشعور بالثقل أو عدم إفراغ الأمعاء بشكل كامل علامة على الإصابة بسرطان القولون. وقد يتفاقم هذا الشعور تدريجيًا مع تقدم المرض.

) إحصائيات سرطان القولون في الدول العربية 

يعد سرطان القولون من أكثر أنواع السرطان انتشارًا في المنطقة العربية ويختلف معدل الوفيات حسب الدولة. وفيما يلي بعض الإحصائيات والاتجاهات:

ا ) معدلات الإصابة: يحتل سرطان القولون المرتبة الخامسة بين أكثر أنواع السرطان انتشارًا في معظم الدول العربية. ويحتل سرطان القولون المرتبة الثانية أو الثالثة في العديد من دول مجلس التعاون الخليجي. ويعد سرطان القولون ثاني أكثر أنواع السرطان شيوعًا في المملكة العربية السعودية ويمثل ما يقرب من ١١٪ من جميع حالات السرطان.

تحتل قطر المرتبة الثالثة في معدل الإصابة بسرطان القولون ويتزايد معدل الإصابة منذ العقد الماضي. وفي الإمارات العربية المتحدة، يحتل سرطان القولون المرتبة الثالثة بين أكثر أنواع السرطان شيوعًا بين الرجال والنساء. وتتمتع مصر بمعدل إصابة أقل بسرطان القولون مقارنة بدول الخليج الأخرى. وفي الأردن، يشكل سرطان القولون ما يقرب من ٩-١٠٪ من جميع حالات السرطان مع معدل إصابة أعلى قليلاً بين الرجال مقارنة بالنساء.

What are the early signs of colon cancer?

Colon cancer or colorectal cancer, which is also known as bowel cancer, is a type of cancer that originates in the colon or the rectum. Colon or rectum are parts of the digestive system and plays a vital role in absorption of fluid and nutrients from the digested food material. Colon cancer usually originates as a small and non-cancerous sessile clump called polyp. This polyp forms in the inner lining of the colon or the rectum and can be precursor of colon cancer. Overtime some polyps may become cancerous.

What are the early sign of colon cancer?

The most common early signs and symptoms of colon or colorectal cancer are:

a) Changes in bowel habits:

Unexplained changes in bowel habits is evident by changes in stool consistency. It is also evident by feeling that bowel does not feels completely empty even after passing the stools.

b) Blood in the stool:

Blood in the stool is evident by passing of bright red blood or dark tarry stools which may further indicate bleeding from the colon. This can be an early sign of colon cancer and be of mild grade in nature but the symptom may gradually worsen or develop.

c) Abdominal discomfort or cramps:

Early signs of colon cancer can be evident by unexplained frequent bloating and cramping pain in the abdomen. Discomfort in the abdomen can also be evident by feeling of fullness or bloating in the abdomen even after eating small amount of foods

d) Unexplained weight loss:

Sudden and unexplained loss of weight more than 10% of body weight without changes in the diet or the physical activity levels. This sudden loss of weight can be due to cancer cells consuming excessive amount of energy.

e) Weakness and fatigue:

Unexplained feeling of fatigue or weakness and it does not go away with rest. This can also be evident by getting up tired from the bed or unable to get up from the bed.

f) Anaemia:

Rectal cancer can also lead to anaemia or low haemoglobin due to bleeding in microscopic amount from the cancerous growth in the colon.

It is most important to note that the above mentioned symptoms can be present in many other health conditions, and does not necessarily means that you have developed stomach cancer. It is strictly advisable to consult your health care professional appropriate diagnosis.

Colon cancer, which can also be called as colorectal cancer it includes cancer which originate in both the colon and the rectum. Colorectal cancer is a type of cancer that originate or begins in the large intestine (colon). Colorectal cancer typically begins as non-cancerous and dysplastic clumps of cells called polyps. This polyp is sessile and forms in the inner linings of the colon or the rectum. Gradually, over time some polyp may acquire additional mutations and can lead to cancerous transformation.

Risk Factors of Colorectal Cancer

The exact cause of colorectal cancer is still unclear but certain factors that increases the risk factors are:

a) Age: Most people with colon cancer are diagnosed above the age of 50 years but it is a well-known fact that it can occur at any age.

b) Genetics: Inherited genetic mutations like Lynch Syndrome and Familial Adenomatous Polyposis (FAP) can increase the risk of colon cancer.

c) Diet: A diet rich in red meat, processed meat, and low on fiber can increase the risk of colorectal cancer.

d) Inflammatory Bowel Disease: Inflammatory Bowel Disease like Crohn’s disease or ulcerative colitis can increase the likelihood of colorectal cancer.

3) Symptoms of Colon Cancer

Colon Cancer at earlier stages may not cause any symptoms but as the disease progresses certain common symptoms may include:

a) Changes in bowel habits: Changes in bowel habits can be manifested by diarrhea, constipation that last for more than a few days. As the disease progresses, this may gradually worsen.

b) Bleeding per rectum: Colon Cancer can also be manifested by bleeding per rectum or blood in the stools. Fecal Occult Blood Test (FBT) is usually done for diagnosis of colon cancer.

c) Unexplained Weight Loss: Loss of more than 10% of weight with not even trying to do so warrants further investigations. Colon Cancer at advanced stage can also be manifested by weight loss.

d) Fatigue or Weakness: Persistent feeling of weakness or fatigue can be sign of colon cancer. This feeling of fatigue or weakness does not even resolve with adequate rest.

e) Heaviness in the bowel: A feeling of heaviness or incomplete evacuation of the bowel can be sign of colon cancer. This can gradually worsen as the disease progresses.

4) What is Ayurveda?

Ayurveda is one of the oldest system of medicine that has originated and progressed in India since 5000 years. The words “Ayurveda” is derived from two Sanskrit words “Ayur”, meaning life, and “Veda” meaning knowledge of life. So Ayurveda translates into knowledge or science of life. Ayurveda strongly emphasizes and focuses on achieving balance between mind, body, and spirit. Key concepts of Ayurveda as follows:

a) Doshas: Ayurveda describes that every person is unique combination of three doshas: 1) Vata. 2) Pitta. 3) Kapha. Vata dosha governs the movement and communication in the body. Pitta dosha governs the digestion and metabolism in the body. Kapha helps to maintain the structure and integrity of an individual.

b) Diet and Nutrition: Ayurveda strongly believes and promotes that wholesome diet with carbohydrates, proteins, and fats is most important for patients suffering from cancer. Ayurveda promotes that food or diet should consist of six rasa’s (Shadha Rasa’s): 1) Madhur. 2) Amla. 3) Lavana. 4) Katu. 5) Tiktha. 6) Kashaya. Each diet of cancer patients and normal person should consist of all the six rasa’s. As per Ayurveda, the person should start eating first with Madhur Rasa, and then should end with Katu Rasa. Ayurveda also promotes against excessive consumption of any rasa’s under the concept of ‘Ati Sarvatra Varjayeta’.

5) Concept of Colon Cancer in Ayurveda

As it is well described that antra (intestine) and guda (rectum and anal canal) is formed from raktha (blood) and kapha after being digested by pitta and vayu. So dusthi of raktha and kapha can be causative factor in colorectal cancer.

In Ayurveda, no such specific term for colorectal cancer is mentioned but its ‘Dosha Samprapti or Etiopathology’ can be compared with the topic or roga called ‘Arbuda/अर्बुद’. When the same dosha and dushya samprapti mentioned in Arbuda/अर्बुद affects Pakvashaya, it is called पक्वाशय and गुद अर्बुद.

It is important to note that the composition and set of doshas (vitiated Kapha and Vata Dosha) which when affects Mansha dhatu causes ‘Arbuda/अर्बुद’, and when the same set of doshas affects पक्वाशय and गुद it causes ‘Colorectal Cancer’ or पक्वाशय and गुद अर्बुद.

6) Treatment of Colon Cancer through Ayurveda

Ayurveda strongly believes and preaches in finding the causative factor of colon cancer and prevent it from originating. The treatment aspect of colon cancer is divided two main parts:

a) Oral Ayurvedic Medications in Colon Cancer. b) Panchakarma in Colon Cancer.

Somanathi Tamra Bhasma is a traditional Ayurvedic formulation that is prepared using purified copper (Tamra), and it is further processed with various herbs and minerals. Bhasma traditionally refers to fine powder that is prepared through a process of purification, calcination, and trituration with herbal plants.

For preparation of Somanathi Tamra Bhasma, Tamra (Copper), Parada (Mercury), Gandhaka (Sulphur), Haratala (Arsenic trioxide), and Manashila (Arsenic dioxide) is taken. The quantity of the individual ingredients are taken as it is: 1) Tamra (Copper), Parada (Mercury), and Gandhaka (Sulphur) is taken into equal amount. 2) Haratala (Arsenic trioxide) is taken half of the quantity of Gandhaka. 3) Manashila (Arsenic dioxide) is taken half the quantity of Haratala. A pisthi is prepeared from the mixture and the mixture is then heated in Valuka Yantra for 36 hours. Later is Yantra is let to cool down and Somanathi Tamra Bhasma is collected.

1.What is Panchakarma?

Panchakarma is one of the most effective detoxification and rejuvenative therapy mentioned in Ayurveda. Ayurveda is traditional system of Indian medicine and is practiced in India since ages. The term “Panchakarma” is derived from or translates to “five actions” or “five treatments”. Panchakarma helps to restore balance of the doshas (Vata, Pitta, and Kapha). The five main procedures of Panchakarma are:

a) Vamana (Therapeutic Emesis): Vamana facilitate in elimination of dustha doshas from the stomach and the respiratory tract. Vamana consist of therapeutically induced vomiting and works mostly on dustha Kapha Dosha. Vamana Therapy helps in purification of kapha Dosha and helps is amelioration of disease related to it.

b) Virechana (Purgation Therapy): Virechana Therapy helps in detoxification or removal of dustha dosha from the ado amashaya region. Virechana Therapy helps to detoxify mostly pitta dosha from the body and ameliorate disease related to it. Virechana consist of induced loose stools and helps to eliminate toxins from the body.

c) Basti (Enema Therapy): Basti Therapy means administration of herbal oils or decoction through anal region. Basti Therapy helps to detoxify the body and balance the vata dosha. Basti Therapy is prominently used to pacify Vata Dosha and ameliorate diseases related to it.

d) Nasya (Nasal Administration): Nasya involves administration of herbal oils, powders, and juice of plants through the nostrils. b) Invasive Ductal Carcinoma (IDC): Invasive Ductal Carcinoma is the most common type of cancer. In this type of cancer originates in the ducts of the breast and spread to other parts of the body.

c) Invasive Lobular Carcinoma (ILC): Invasive Lobular Carcinoma type of cancer start in the lobules. Lobules are the gland that produces milks. This cancer can spread to other parts of the body.

d) Triple-Negative Breast Cancer (TNBC): Triple-Negative Breast Cancer is a type of breast cancer that does not have any of the three common receptors: 1) Estrogen Receptor (ER). 2) Progesterone Receptor (PR). 3) HER2/neu. This is more aggressive variant of breast cancer and hard to treat

e) HER2-Positive Breast Cancer: HER2-Positive Breast Cancer tests positive for a protein called human epidermal growth factor receptor 2 (HER2), which promotes the growth of cancer cells.

3) Role of Panchakarma in Treatment of Breast Cancer

a) Vamana

It is well established fact that when breast cancer is detected at earlier stages then surgical resection is best known option for the patient. If the breast nodule is unresectable due to some unavoidable anatomical locations or if the patient is suffering from cardiac or renal ailments then Radiotherapy or Chemotherapy is required to be planned. Mrudu Vamana Therapy (Mild) can be planned before chemotherapy or radiotherapy for better results.

In post-surgical cases, adjuvant chemoradiotherapy is generally planned so after Vamana, Virechana can be planned for better results.

Virechana can also be planned in breast cancer cases who are post-surgical cases so as to prevent a relapse. Virechana helps in kostha shudhi and so further proves beneficial in proper absorption of rasayana dravyas.

c) Basti

In breast cancer patients who are debilitated by the disease and are suffering from cardiac ailments or multiple co-morbidities, Basti Therapy can be planned. For Koshtha Shuddhi of breast cancer patient with an increased risk Shodhaniya Basti is more viable option than Virechana.

For breast cancer patients who are suffering from extreme weight loss due to Raktha (Blood) and Mansa (Muscle) Kshaya (Loss), Yapana Basti can be planned. Yapana Basti is one the best treatment modality in cachexic breast cancer patients who needs to gain weight and to improve general health.

Matra Basti with Bala Oil can also be planned in breast cancer patients as it is easier to administer and does not require purva and paschata karma of basti to be followed. Ashwagandha Oil helps the cancer patient to gain weight and improve the raktha (Blood).

4) Lepa or Local Application

Different lepa or medications for local application is mentioned Ayurveda. Certain Lepa which are usna and thiksna in nature can help to resolve breast cancer. This lepa helps to resolve the inflammatory stage, the same in Ayurveda terms can be described as it helps to bring pakva awastha in apakva doshas.

Dashang Lepa (दशांग लेप) in Breast Cancer

Dashang Lepa is prepared by mixing powder of Tagara (Valeriana wallichi), Kushta (Saussurea lappa), Rakta chandana (Pterocarpus santalinus), Ela (Elettaria cardomum), Haridra (Curcuma longa), Yashtimadhu (Glycyrrhiza glabra), Daruharidra (Berberis aristata), Jatamanshi (Nardostachys jatamansi), Sugandha bala (Coleus vettiveroides) and Shirisha (Albizzia lebbeck).

As we all know Dashang Lepa is easily available formulation and in Ayurveda is mentioned in Vranashopa Disease (Inflammatory condition due wound). It helps in Vranashopa due to stanik dosha pachana. 1) What is breast cancer?

Breast cancer is a type of cancer that starts with abnormal and uncontrolled growth of cells in the breast tissue. It can start with one or both breasts. Most commonly seen in women, rarely in men. Breasts are two soft glandular organs on the upper front chest of our body. in this article, we also discuss the  Ayurvedic diet in breast cancer.

The breast is made up of following different parts:

a) Skin: The skin is made up of the nipple and areola. The nipple is an area at the center of the areola with an opening to release milk (in women after childbirth). The areola is a circular darker colored area of skin surrounding the nipple.

b) Lobes: Each breast has 15-20 sections known as lobes. Small sections of tissue within then lobes that contains gland which produces milk are called a lobules.

c) Glands: The breast contains glandular tissue made up of lobules or small sections of tissue that contain tiny bulb like glands which produces milk ducts are the tubes that connect lobules, lobes and bulbs that lead to the nipple.

d) Fat: The breast is made up of fatty tissue or adipose tissue, which fills the spaces between the lobules and ducts. The ratio of fat to glandular tissue can change depending on a woman’s age.

e) Ligaments: Dense bands of connective tissue called ligaments that supports the breast.

f) Blood vessels– The breast receives blood from multiple vessels like internal thoracic artery, axillary artery and thoracoacromial artery. The breast skin receives blood from subdermal plexus.

g) Lymph vessels– The breast also contains lymph vessels that carry lymph fluid to lymph nodes. They are found in clusters in the armpit, above the collarbone in the neck and in the chest.

2) Types of breast cancer:

There are two main types of breast cancer- Invasive type and Non-Invasive type 

Invasive breast cancers are-

a) Invasive (infiltrating) ductal carcinoma (IDC): This is most common type of breast cancer that starts in the milk duct and spreads to nearby breast tissue. Seen in 70-80% cases

b) Invasive lobular carcinoma: This is second most common type of invasive breast cancer that begins in the cells lining the lobules and spreads to nearby breast tissue.

Non-invasive breast cancers are-

a) Ductal carcinoma in situ: Like IDC, the breast cancer starts in your milk ducts. This cancer does not spread beyond your milk ducts.

b) Lobular breast cancer in situ: The breast cancer starts in cells that lines lining the breast lobules, the glands that produces milk. It is rarely seen. Most likely to affect both the breasts.

The non-invasive breast cancers are not life threatening and generally confined to the area they are originated, but can turn into invasive ones if left untreated.

Other types of breast cancers includes inflammatory breast cancer, Metastatic breast cancer, lobular carcinoma in situ and triple negative breast cancer.

3) Signs and Symptoms of Breast cancer:

a)  Generally breast cancers are benign tumors. Symptoms include a lump in the chest that is a thickened area of the skin which feels different from the surrounding tissue, a lump in the armpit, pain in chest, bloody discharge from the nipple. Changes in the size, shape or appearance of the breast.

b)   A nipple that looks flattened or turned inwards.

c)  Changes in the skin over the breast such as skin that looks dimpled or looks like orange peel. Scaling, peeling, crusting or flaking of the skin on the breasts.

4) Concept of Ayurveda-

‘Ayu’ means lifespan and ‘Veda’ means science. It is system of traditional Indian medicine that believes in natural approach in all aspects of health and well-being. It deals with diagnosis based on tridosha, saptadhatu, panchmahabhuta, dashavidha roga pariskha and uses herbal and herbo-mineral drug combinations along with panchkarma therapy. It has following concepts-

Tridosha concept:

There are three doshas or bodily humors -Vata, Pitt and Kapha that governs physiological and psychological processes in the body. Ayurveda believes that balance of doshas is health and an imbalance results in disease. Imbalance in Vata causes constipation, anxiety, dry skin, constipation, cognitive disorders. Imbalance of Pitt causes burning sensation, peptic ulcers, excessive body heat and anger, imbalance of Kapha causes obesity, sinus congestion, diabetes, cough etc.

Panchmahabhuta Concept

There are five natural elements- Earth, water, fire, air, and space. Everything in the universe including our body and mind is made up of these five elements known as panchmahabhuta. This concept is very much useful in nidan (diagnosis) and chikitsa (treatment) of various disorders for the aim of Dhatusamya (balance e of fluids in our body). A combination of each of the five element known as panchmahabhuta makes up for three doshas.

5) Types of chikitsa-

There are three types of chikitsa-

a) Daiva-vyaprashray which is divine therapy, a non-pharmacological approach.

b) Yukti-vyapashray chikitsa which is using logical reasoning and planning to treat patients.

c) Satvavajaya chikitsa which means mental restraint, psychotherapy or mind control therapy. Traditional mental health promoting practices includes Swastavritta (good health regimes), Sadavritta (good conduct) and yoga.

The therapeutic principles of Ayurveda is based on pacification of doshas, potentiation of dhatus, functioning of Agni, cleaning the obstruction and preventing formation of Ama.

The word panchakarma means ‘five actions’. There are five major treatment methods like vamana, virechana, nasya, basti and raktamokshana that helps to detoxify our body and helps in restoration of imbalanced doshas. It is effective in stimulating metabolism, boosting the immunity, unblocking energy, rejuvenating cells and tissues and relieving stress.

It is a practice used for centuries whose origin is seen from the period of Rigveda and Atharvaveda. Yoga sadhana is for promoting physical, mental and spiritual well-being. Yoga includes practicing certain asanas, meditation and mudras. It emphasizes on proper breathing techniques, proper postures, proper relaxation with dhyan-dharana and positive thinking. It establishes connection between body, breath and mind and builds strength and flexibility. This approach helps in better healing from a diseased state.

8) Concept of Oja-

Oja is a Sanskrit word which means ’vigor’ or ‘essence of vitality’. It is essence of all sapta dhatus (7 tissues). It is explained as’ Bala’ which is strength. It is the vital energy that rules our immunity, strength and happiness. In cancer patients particularly Oja kshay is seen at a vast extent.

9) Role of diet in cancer management-

A well balanced diet may help reduce the risk of malignancy, provides better tolerance to treatment and its side effects, helps in better recovery, keeps up energy and vitality in cancer patients.

There are two perspectives in case of cancer patients-
  1.  Research has shown that obesity is one of the risk factors for 13 types of cancers like cancer of breast, colorectal, esophagus, liver etc. The chronic inflammatory state in an overweight person is linked with lesser life expectancy and increase in chances of complications along with other metabolic disorders like diabetes mellitus and cardiovascular diseases. Obesity also increases the risk of those dying from cancer and influences treatment choices. Hence keeping a healthy weight is very important.
  2. Other aspect is cancer and its treatment can sometimes negatively affect your appetite, making it difficult to have a balanced diet. It may lead to cachexia which is associated with severe weight loss, muscle loss and fat loss.
Diet and nutrition is an integral part of cancer related treatment. A healthy nutrient dense diet is very important. Ayurvedic texts have detailed discussion about concept of ‘Aam’. Aam denotes the abnormal and impaired process of digestion and metabolism that leads to build up of toxic byproducts which cannot be neutralized or eliminated by the body. In Ayurvedic view, all diseases including cancer are originated from this ‘aamdosha’ . Aamdosha is formed due to malfunctioning of ‘Agni’ that is digestive fire. To correct Agni, proper diet management is a crucial step. Modern dietary regime for breast cancer-
  • Eating plant-based diet.
  • Keep small and frequent meals.
  • High protein high fibre diet.
  • Limit sugar and fats.
  • Eating variety of colorful fruits and vegetables.
  • Limit caffeine.
  • Optimum hydration by drinking water.
  • Stop processed and cured foods.
  • Rainbow diet.
  • Stop tobacco and alcohol use.
  • Organic foods.
  • Anti-Estrogenic herbs.
Ayurvedic Dietary recommendations for breast cancer –
  • Focus on wholesome foods- This category includes whole grains like cereals, millets and pulses which are great source of complex carbohydrates. According to ayurveda they come under Shukdhanya and Shamidhanya. It is advised to eat one year aged rice (old rice) mainly from Shashti shali subtype. Yellow moong dal is considered to be best among pulses as it is a good source of complex carbohydrates, protein and easier to digest so optimum for cancer patients who are struggling to eat and has very low appetite.
  • Plenty of vegetables. There is detailed description of vegetables under shaka varga. Among them gourd family and cruciferous vegetables are said to be good for cancer patients.
  • Milk- Low fat milk and milk products are beneficial for cancer patients. Low fat milk includes cow’s milk and goat’s milk.
  • Ghrita- Ghee is rich source of fat soluble vitamins A, D, E and K. Also a source of omega-3 fatty acid which has antioxidant and anti-inflammatory effects. In ayurveda different disease specific medicated ghritas (ghee) are advised.
  • Mansa varga- Mutton soup and chicken soup is recommended, good for bone and muscle strength.
  • Pathya Kalpana- Pathya Kalpana is preparing food to treat diseases, make it more fruitful and curative. These preparations increases digestive capacity, increases nutritional value of food. They include manda, peya, vilepi yusha and yavagu. Manda (liquid gruel), peya (thin gruel), vilepi (thick gruel) is made from rice while yusha (soup) and yavagu are made from green and yellow green gram.
  • Flax seeds- It is called as ‘alasi’ in ayurveda. It helps to stimulate breast milk production post-partum. From this we can say flax seeds have action on milk producing glands and cells. Lignin’s a type of phytoestrogen and flax seed oil can change estrogen metabolism hence believed to potentially reduce breast cancer.
  • Turmeric- Curcumin found in turmeric is said to have anticancer and chemo-preventive effects in breast cancer. In ayurveda it is called as ‘Yoshitpriya’ means dear to women. It is used widely in treating pcos, also improves insulin resistance. From this we can say it turmeric has action on ovaries.
  • Avoid simple sugars, processed foods, canned foods, aerated drinks, junk foods, stale foods, caffeine, sour and pungent foods.
  • Avoid alcohol and tobacco
Individualized calorie intake should be taken into consideration along with its digestion capacity. Hence quantified diet plan is to followed according to conditions like obesity or cachexia in order to maintain healthy weight and better recovery from cancer and cancer related treatment.

What is Panchakarma treatment as per Ayurveda?

Ayurveda is a traditional system of Indian Medicine and Panchakarma is one of its important treatment modality. Panchakarma consists of series of detoxification and rejuvenation therapies that helps to ameliorate vast range of diseases and treat its root cause.

The term “Panchakarma” is derived from or translates to “five actions” or “five treatments”. These five different treatment modalities in Panchakarma helps to cleanse the body of the dustha doshas. Panchakarma helps to balance doshas (Vata, Pitta, and Kapha) of the body and promote well-being of an individual.

The five main treatments of Panchakarma are:

a) Vamana (Therapeutic Vomiting): Vamana is mentioned for cleansing or removing dustha doshas from head and neck region, and also from urdhava amashaya. Vamana helps to purify dustha kapha dosha from the body and also helps to ameliorate ailments related to it. Vamana involves induction of controlled vomiting and helps to clear mucus and toxins from the respiratory and the Gastrointestinal Tract.

b) Virechana (Purgation Therapy): Virechana is mentioned for cleansing or removing dustha doshas from ado amashaya region. Virechana helps to purify pitta dosha from the body and helps to ameliorate ailments related to it. Virechana involves induction of controlled loose stools and helps to eliminate toxins from the body.

c) Basti (Enema Therapy): Basti Therapy is mentioned as a therapeutic enema consisting of herbal oils or decoction. This herbal oil or decoction helps to cleanse the colon and further helps to purify Vata Dosha and ameliorate diseases related to it.

d) Nasya (Nasal Administration): Nasya involves administration of herbal oils or powders through the nose. It helps to clear doshas from the urdhavagata region that is nasal passages, sinuses, and head region.

e) Raktamokshana (Bloodletting): Raktamokshana involves removing small amount of blood from the veins and this helps to detoxify the body. Raktamokshana helps to purify the blood in the body and helps in amelioration of diseases related to it.

Panchakarma is usually customized and tailored as per individual constitution of doshas (Prakriti), General Health or Bala of that individual, and specific imbalance of doshas from which the patient is suffering from. Panchakarma should always be done under the guidance of Ayurveda Practitioners.

What is Lung Cancer?

Lung cancer is a type of cancer that originates in the lungs. Lungs consist of two spongy organs and is located within the chest. Lungs plays one of the most vital role in the body and facilitates exchange of gases. It helps in the absorption of oxygen and expelling carbon-dioxide.

Lung cancer is one of the most common type of cancer and has a significant contribution in mortality due to cancer. Lung cancer is broadly classified into two main type based on the histopathological appearance:

a) Non-Small Cell Lung Cancer (NSCLC): This is the most common type of lung cancer and accounts for almost 85% of all lung cancer cases diagnosed. Non-Small Cell Lung Cancer (NSCLC) has several subtypes which includes:

  • Adenocarcinoma: It is usually found in outer parts of the lungs and is more common in non-smokers.
  • Squamous Cell Carcinoma: It is usually common in central part of the lungs and more common in smokers.
  • Large Cell Carcinoma: It can originate in any part of the lungs and tends to spread more rapidly.

b) Small Cell Lung Cancer (SCLC): Small Cell Lung Cancer (SCLC) is a less common type of lung cancer and accounts for almost 15% of all the cancer cases diagnosed. Small Cell Lung Cancer (SCLC) tends to spread more rapidly and are aggressive in nature.

Role of Panchakarma in Treatment of Lung Cancer

a) Vamana:

It is well established fact that when lung cancer is detected at earlier stages then surgical resection is best known option for the patient. If the lung nodule is unresectable due to some unavoidable anatomical locations or if the patient is suffering from cardiac or renal ailments then Radiotherapy or Chemotherapy is required to be planned.

Mrudu Vamana Therapy (Mild) can be planned before chemotherapy or radiotherapy for better results. For general ayurvedic treatment of lung cancer it’s better to evaluate the risk hemorrhage before Vamana is advised and if given, it should always be Mrudu Vamana.

b) Virechana

Virechana is one the most important treatment modality in Lung Cancer as it helps in purification or detoxification of doshas that are causative factors in lung cancer. Prior initiating the Chemotherapy or Radiotherapy to lung cancer patients, Virechana Therapy can be planned so to have better treatment response.

After surgical treatment of lung cancer, Rasayana Therapy needs to be planned to prevent relapse of lung cancer. Virechana Therapy helps to detoxify or remove the dustha dosha from the body and causes in agni deepana (increase digestive power). Virechana Therapy leads to Kostha Shuddhi and helps in better absorption of Rasayana Dravya’s. After Virechana Therapy, different Rasayana like Vardhamana Pippali Rasayan, Kushamanda Rasayan can be planned.

أعراض السرطان الرئوي .
